Affinity Health Group Recognized by NCQA

Eight of Affinity Health Group’s primary care practices are recognized on Achievement of Recognition for Systemic Use of Patient-Centered, Coordinated Care Management Processes (2017 PCMH Recognition) from the National Committee for Quality Assurance (NCQA) for providing the high quality of care required by Patient-Centered Medical Home™ (PCMH) standards.

The Patient-Centered Medical Home is a model that was created to place the patient at the forefront of their care. PCMHs promote partnerships between the patients and their physicians.

  • Reduce the Costs of Care: Identify potentially avoidable hospitalizations and emergency room visits, and deliver preventive care following evidence-based practice guidelines.

NCQA’s mission is to improve the quality of health care. The recognition program is a collection of guidelines built from evidence-based practice focusing on the patient. The recognition program was developed to help clinicians and practices support the delivery of high-quality care.
