Affinity Health Group has made it easier and more convenient to request your medical records. Through the link below, you or your authorized representative can request records easily and securely online.

This process allows you or your physician, family member or attorney, if authorized, to request the records needed for your specific situation. Seeking a second opinion? Moving out of state? We will help you obtain what you need quickly, conveniently and confidentially. The process takes five to 10 minutes to complete.
